Why You Have Acne

Acne often forms from an overproduction of sebum (the oily substance on the skin) and bacteria (known as P. acnes).
What Causes Acne?
When pores become clogged with sebum, bacteria can rapidly grow and whiteheads (clogged pores that have reached the surface), blackheads (clogged pores which have reached skin’s surface and darkened due to bacteria’s oxidation), and pustules (clogged pores which remain below the surface) begin to appear.

Whereas some produce little oil that's the sebum, others may produce too much oil on the face. We can't do much about how much oil our skin produces but we can mitigate the breakouts and in some cases, eliminate them.

Tretinoin and Accutane

The most common medication for acne is the Tretinoin. Tretinoin is also known as retinoic acid. It's the generic name for synthetic vitamin A. For acne, it is applied to the skin as a cream, gel or ointment. This retinoid is different from The Ordinary Granactive retinoid in terms of strength.

Accutane (isotretinoin) is a retinoid used for the treatment and prevention of severe acne. A difference is Acutane is an oral drug, while Retin-A is a topical one (applied).Isotretinoin works by decreasing the size of the sebaceous glands in the skin. The sebaceous glands are responsible for creating sebum, the oil in skin that can clog pores and cause acne. When the sebum is controlled, breakouts stop. It kills the bacteria in your pores that can cause acne and inflammation.

Accutane can cause miscarriage, premature birth, severe birth defects, or death of a baby if the mother takes this medicine at the time of conception or during pregnancy. Even one dose of isotretinoin can cause major birth defects of the baby's ears, eyes, face, skull, heart, and brain.

Whether to use oral or topical depends on what your dermatologist will recommend. Both are prescribed and not over the counter.

Other acids you can use are Beta Hydroxy Acid, Alpha Hydroxy acids among other mild ones.

The only Beta hydroxy Acid for skin is salicylic acid.
